Cardiovascular-kidney-metabolic (CKM) syndrome is a distinct epidemiological and prognostic disease characterized by the co-occurrence of cardiovascular disease, chronic kidney disease, and type 2 diabetes. Approximately 90% of adults in the U.S. are at risk for or have CKM syndrome, making early recognition and longitudinal assessment essential to preventing irreversible disease progression. As frontline providers treating patients across the full spectrum of care, physician associates are uniquely positioned to aid in detection and management of CKM.

Created with support from Bayer, AAPA’s new Cardiovascular-Kidney-Metabolic Syndrome Toolkit is designed to help both providers and patients recognize CKM risk factors, as well as understand prevention, screening, staging, and treatment.

Resources for Providers
The toolkit includes tools designed specifically for healthcare providers. With new 2026 guidelines now available, refresh your clinical knowledge on the pathophysiology of CKM, factors that influence risk, how to apply a stage-based screening framework, integrate PREVENT and KDIGO risk stratification, and more.

Resources for Patients
As you care for patients at risk for or navigating CKM, the toolkit includes additional resources they can explore. Through educational materials, a comprehensive patient hub, and a patient-friendly, illustrated guide, patients can better understand their risk for CKM and practical steps to improve their health.

The Cardiovascular-Kidney-Metabolic Syndrome Toolkit is now available in AAPA’s Clinical Resource Library.
