Cardiovascular-kidney-metabolic (CKM) syndrome is a distinct epidemiological and prognostic disease characterized by the co-occurrence of cardiovascular disease, chronic kidney disease, and type 2 diabetes. Approximately 90% of adults in the US are at risk for or have CKM syndrome. With new 2026 guidelines now available on the prevention, detection, and management of CKM, this toolkit is designed to help providers and patients recognize risk factors and understand prevention, screening, staging, and treatment to encourage longitudinal assessment and prevent irreversible disease progression.
