Lifestyle Medicine and the Older Adult

Learning objectives:
- Identify valid measures of successful aging and evaluate how lifestyle interventions contribute to improved health outcomes in aging populations
- Apply evidence-based lifestyle medicine interventions (including sleep, physical activity, nutrition, and social connection) to the management of chronic conditions in older adults to improve mood, function, and quality of life.
- Develop interdisciplinary, patient-centered care plans for older adults with complex conditions by incorporating the 5Ms framework (What Matters, Mentation, Mobility, Medications, Multicomplexity) and connecting patients to appropriate community and behavioral health resources.
