Employment Exit Checklist

The following checklist suggests points to cover when ending an employment sistuation.

Professional Issues

Ö Notify other providers/referrals.
Ö Transfer care for patients in active care to another provider.
Ö Notify patients.
Ö Complete all your medical records.
Ö Notify hospitals, nursing homes, and other places of practice.

Legal/Insurance Issues

Ö Notify state licensing board or department regarding changes in supervising physician.
Ö Notify professional liability insurer.
Ö Review malpractice coverage. If claims-made policy, is employer providing tail coverage?
Ö When does health and other insurance terminate?
Ö Discuss COBRA options for continuing health insurance.
Ö Review contract/formal agreement for restrictions on future practice (such as restrictive covenants).

Benefits/Business Issues

Ö When will you receive final paycheck?
Ö Are you entitled to vacation pay? Sick time payout?
Ö Review your retirement benefits. Be aware of deadlines.
Ö Can you transfer your retirement funds to another fund?
Ö Are you entitled to a bonus or profit sharing? If so, when can you expect payment?
Ö If moving, leave a change of address for future correspondence.

Reference Issues

Ö Discuss practice policy regarding references.
Ö Will your supervising physician/practice be a good reference? If so, get a letter for future use.
Ö Review personnel file and make copies of all reviews.
Ö Review credentialing files.
